Iowa Hawk Application Information

The Healthy and Well kids in Iowa (HAWK-I) program is an initiative by the state of Iowa to provide health care coverage for kids in families with low income. The program aims at making sure that all residents of Iowa have access to quality healthcare regardless of whether they can afford it or not.

The program is sponsored and administered by the Iowa department of health, which accepts applications on a continuous basis? HAWK-I is useful for families which have income that is too high to qualify for MEDICAID but also too low to afford private health care coverage.

Depending on the income level some families pay nothing while others pay between $10 and $40 on a monthly basis.

The program does not charge any fee too Native Americans who have proof of Indian blood or any established tribal affiliations with Indians.

In terms of income qualification, 20% of income is not counted when making comparisons on the various income levels. However this only applies to "earned income" and not applies to unearned income like child support, social security of unemployment income.

Some of the services afforded by the healthy and well kid Iowa include: doctor visits, outpatient services, speech therapy, medical equipment, hearing examinations, inpatient services, and eye care among others.

Eligibility Requirements

In order to qualify for the Healthy and Well Kids Iowa program one needs to meet a set of eligibility requirements. These eligibility requirements include:

  • The applicant must be below 19 years of age
  • The applicant should not have other health insurance coverage
  • The applicant should be a citizen of US, or a legal alien
  • Individuals who qualify for Medicaid cannot qualify for HAWK-I

There are several ways through which one can apply for the Healthy and Well Kids Iowa program, these include:

Paper Application

One can obtain and fill out a paper application from the department of human services by visiting the nearest Department of Human Services office. At this office you will meet a HAWK-I representative who will provide answers to any queries you have regarding the application process and processing.

Once you have the necessary information, you will then proceed to fill the form making sure to provide responses to all questions and to attach any supporting document. Once all these processes have been achieved you should submit the application for processing.

Online Application

A more convenient way of making an application for the Healthy and Well Kids Iowa program is by filling out an online application form. This is much easier and much convenient as one can fill out an application at any time, anywhere.
