Alabama ALL Kids-Children's Health Insurance is an initiative by the State of Alabama that provides healthcare coverage for children and teenagers within the State of Alabama. The beneficiaries of this initiative are usually from families whose income is higher than the Medicaid eligibility set amount, but still lower than 300% of the Federal Poverty Level. The Alabama ALL Kids-Children's Health Insurance is administered by the Alabama Department of Public Health. This initiative provides an array of services some of which include dental care, eye care, prescriptions, physician services, etc.

ALL Kids is a low cost and comprehensive insurance healthcare program for children below the age of 19. With this healthcare plan, families have various payment options, i.e. families can either make income based premiums or make co-payments. Eligibility Requirements

  • Not be residing in an institution.

There are four main ways of application; online application, personal application, application via phone and application via mail/fax.

Online Application

The online application process for the Alabama ALL Kids program can be done by clicking here where one will obtain the application form, fill it out and submit it to the relevant authority.

Personal Application

This form of application is done by visiting any County Health Department or health and community agency and picking up the Alabama ALL Kids application. After filling it out with the relevant information, one can submit it to the relevant authority and find out whether or not they qualify for the program.

Application via Phone

Alabama ALL Kids has a toll free line that interested applicants can call and request for the application form. The number is 1-888-373-KIDS (5437). After calling this number, the members of staff will mail or email you the application form and answer any questions you may have regarding the application process.
