WIC Application Instructions

Once you have filled and submitted the form, you will be contacted by a program representative who will schedule an interview with you. Before going to the interview you should remember to carry important verification documents such as income statement, identification documents and proof of residence.

During the interview you will be subjected to a medical examination which is done free of charge, the program representative will then proceed to verify important documents and ask you questions directed at establishing your eligibility to the program.

Upon successful completion of the process you will be notified of whether you have qualified or not qualified for the program. If you have qualified for the program, you will start receiving the program benefits within 30 days from the time you submitted your application.

Eligibility Requirements

West Virginia WIC- Nutrition Risk Eligibility Requirement

In order to be eligible for the program, applicants must be medically examined by a healthcare professional and proven to be nutritionally at risk. Cases of nutrition risk include: low iron levels, premature delivery and inadequate dietary intake.
