WIC Application Instructions


You can easily apply for the South Dakota WIC program in two easy steps. First you need to contact and book an appointment for an interview with your local South Dakota WIC office available on this link: www.doh.sd.gov/family/wic/locations.aspx (South Dakota WIC list of local offices).

After you schedule an appointment for an interview you should go ahead and fill the application form available on this link: www.doh.sd.gov/family/assets/WICincome.pdf (South Dakota Department of health family card), and make sure you carry all the required documents to the interview as specified by the South Dakota WIC program representative.

On the day of the interview you will undertake a nutritional examination, after which a South Dakota WIC staff will verify your documents. Before ending the interview, the South Dakota WIC program representatives will ask you questions aimed at establishing your eligibility to the program.

Upon successful completion of the application process you will start participating in the South Dakota WIC program and begin to enjoy the benefits of the program.


Eligibility Requirements

Applicants for the South Dakota WIC program need to fulfill a set of eligibility requirements. These program eligibility requirements are broadly divided into:

  • Categorical eligibility requirements
  • Income eligibility requirements
  • Residential eligibility requirements
  • Proof of nutritional risk

South Dakota WIC-Categorical Eligibility Requirements

Applicants to the South Dakota WIC need to belong to the three categories of Women, infants and children. In addition to this, they also need to fulfill the individual categorical eligibility requirements discussed below:

Women

Women applicants for South Dakota WIC program need to be:

  • Undergoing pregnancy or within 6 months after delivery of the child
  • Breastfeeding up to the infant's first birthday
  • Postpartum

Infants

To be considered an infant a child needs to be less than one year of age

Children

Children up to five years old, qualify for the program

South Dakota WIC -Income Eligibility Requirements

In order to qualify for the South Dakota WIC program, applicants need to meet a specified set of income eligibility requirements. Applicants receiving benefits from Medicaid, TANF, SNAP or other social services program are considered automatically income eligible.

South Dakota WIC-Residential Eligibility Requirements

In order to qualify for the South Dakota WIC program, you must be a resident of the state of South Dakota and be a US national or a legal alien.

South Dakota WIC - Proof of Nutritional Risk

Applicants to the South Dakota WIC program are required to undertake a medical examination done by a qualified health care professional and proof that you are nutritionally at risk.
