WIC Application Instructions


Apply by Phone: In order to apply for Oklahoma WIC, you will have to call your nearest WIC clinic. You can find contact information and directions for your nearest Oklahoma WIC office here: www.ok.gov/health2/documents/WIC.pdf

You can also call the State Department of Health toll-free at: 1-888-655-2942.

When you call Oklahoma WIC clinic, you will get an appointment. A representative will interview you and decide whether you meet eligibility guidelines for the program or not.

When going for an appointment, you will have to take your WIC folder, proof of income, proof of other services if you receive, and children immunization record.


Eligibility Requirements

In order to qualify for Oklahoma WIC, you will have to meet these basic eligibility requirements:

  • Be a U.S citizen or a qualified alien
  • Be a resident of Oklahoma
  • Be on a nutritional risk
  • Meet income guidelines

Once you meet these requirements, you will have to meet categorical requirements for the program. The following can receive benefits through Oklahoma WIC:

  • Pregnant women
  • Breastfeeding women
  • Postpartum women
  • Infants up to age of 1 year
  • Children up to age of 5 years

Income Eligibility

In addition to meeting general and categorical requirements, applicants will also have to meet income requirements to receive benefits through Oklahoma WIC. NOTE: Those who receive TANF, Medicaid, or SNAP benefits will automatically clear income eligibility. They don't need to be determined for it.
