WIC Application Instructions


To apply for Ohio WIC benefits, you will have to fix an appointment with your local WIC clinic. You can call WIC office and fix an appointment or visit in person to get a date for appointment. Interview Information

When you are going for your appointment, make sure that you are taking all the necessary documents with you. You will have to take proof of identity, proof of residence, and proof of your income to provide it during eligibility screening process at the interview.


Eligibility Requirements

Ohio WIC is only available to:

  • Women (pregnant, postpartum, breastfeeding)
  • Infants under the age of 1 year
  • Children under the age of 5 years

The applicants will have to:

  • Be residents of Ohio
  • Meet income guidelines
  • Be on a nutrition risk as determined by a health professional
