CHP+ Application Instructions


Apply in Person

To apply in person, you will have to locate local office of Department of Human Services (DHS). You can visit the office and obtain an application from the representing agent. Complete it and hand it back to the representative in order to apply for CHP+.

Apply by Mail

In order to apply for CHP+ program through mail, you will be required to download and complete an application. You can download the application from links below

Once you have downloaded the application form, fill it out providing all the necessary details. If you need any help while completing the application, you can contact at (800) 359 1991 to get assistance with application.

After you complete your application, you can mail it to your local county office. You can also use this tool to find an application assistance site close to you.


Eligibility Requirements

In order to receive benefits through CHP+ program, children will have to meet the following eligibility criteria

  • Must be under the age of 19 years
  • Must be a U.S national or a qualified alien
  • Must be a resident of state of Colorado
  • Not have any other health insurance including Medicaid

Income eligibility

In order to qualify for the program, children will have to meet all the general requirements mentioned above. In addition to those general requirements, children will also have to meet additional income requirements. The income limits are set according to family size. This means that you will first determine your household size and then compare your income against your household size in the income chart available chart available here.

In order to measure your income eligibility accurately, you will be required to deduct certain expenses from your income before you use it to determine eligibility. Such expenses include child care and elder care costs, child support payments, medical costs, and alimony payments. You will calculate expense for each of these and then subtract it from your gross salary.

When calculating the household size, make sure that you count a pregnant woman as two persons for the purpose of CHP+ program.
